Rehabilitation Phases
Phase I: Protection & Passive Motion (Weeks 0-4)
Goals: protect repair, control pain/swelling, begin passive/assisted motion.
Phase II: Active-Assisted Range of Motion (Weeks 4-8)
Goals: active motion, rotator cuff isometrics, scapular stability.
Phase III: Strengthening & Stability (Weeks 8-16)
Goals: strength > 70%, sport-specific conditioning begins.
Phase IV: Return to Sport / Work Conditioning (Weeks 16+)
Goals: full strength, return to throwing/overhead work, performance restoration.

When can I start lifting my arm overhead again?
That’s a great question — and one we get often. Everyone’s recovery is unique. The key is to let your shoulder heal and follow the gradual progression outlined in your SIGMA recovery plan. Overhead motion comes naturally as strength returns — safely and at the right time.
When can I drive again?
Driving depends on comfort, strength, and safety — especially your ability to react quickly. Once you’re off pain medication and can move your arm confidently without hesitation, we’ll guide you on when it’s appropriate to resume.
Can I sleep on my side?
Early on, it’s best to protect the repair and keep pressure off your shoulder. As healing advances, your therapist will help you transition to more natural sleeping positions using supportive pillows for comfort and alignment.
When can I go back to the gym or exercise?
Returning to the gym happens in phases. We’ll reintroduce motion and strength gradually so your shoulder builds endurance without risking reinjury. You’ll get there — with precision and patience.
Do I still need to use the sling?
Your sling is your safety net early on. As your healing progresses, you’ll use it less under our guidance. We’ll let you know exactly when it’s time to begin weaning from it.
How do I know if my recovery is on track?
Progress is measured by function, not speed. Through your SIGMA Scoreboard and ongoing check-ins, we monitor your motion, strength, and comfort so you can see your recovery in real time.
Why does my shoulder feel tight or stiff?
That’s normal during early healing. Gentle movement, stretching, and consistent therapy help restore mobility. Stiffness is part of the process — not a setback.
When can I return to work?
This depends on the type of work you do. Desk jobs allow earlier return; physical jobs require longer protection. We’ll tailor your plan based on your demands and recovery milestones.
Will I regain full motion and strength?
